If you need to reschedule, we kindly ask you to provide 48-hour notice so we can offer the appointment to another patient.

What to bring to your first visit
- Photo ID and insurance card (if any)
- Medication list and allergies
- Dental X-rays (if taken in the last 12 months) or your previous dentist’s info so we can request them
- Parent/guardian for minors
